Medical Form

At Ilchester Community Primary School, the health and well-being of our students are of utmost importance. We are committed to providing a safe and supportive environment where all medical needs are addressed promptly and effectively. To ensure we can provide the best care for your child, we ask parents and guardians to follow these guidelines regarding medical concerns and the completion of medical forms:

Medical Concerns

  • Communication: Please inform the school of any medical conditions or concerns that may affect your child’s well-being or learning. This includes allergies, chronic conditions, and any recent illnesses or injuries.
  • Medication: If your child requires medication during school hours, please provide the medication in its original packaging, along with clear instructions for administration. All medications must be handed to the school office by an adult.
  • Emergency Contacts: Ensure that the school has up-to-date emergency contact information. This is crucial in case we need to reach you quickly.

Filling in Medical Forms (found below)

  • Medical Information Form: At the start of each school year or when your child enrols, you will be asked to complete a medical information form. This form collects essential details about your child’s health needs and any medications they may require.
  • Permission to Administer Medication: If your child needs to take medication at school, you must complete a permission form authorising school staff to administer the medication. This form should include dosage instructions and any specific requirements.
  • Allergy and Asthma Plans: For students with allergies or asthma, we require an action plan from your child’s healthcare provider. This plan should outline the steps to take in case of an allergic reaction or asthma attack.
